Provide pharmaceutical care for patients, monitor drug therapy and coordinate drug distribution, ensure medication safety, participate in education of students and/or residents, and develop and maintain professional relationships with other healthcare providers. Provide pharmaceutical care to hospital patients as a member of the pediatric pharmacist teamReview new medication orders for appropriateness of therapy: including patient assessment, identification and documentation of drug therapy problems and resolutions, review of appropriateness of therapy and follow-up and discharge counseling for selected patientsMonitor patients on an ongoing basis and adjust medications and doses after consulting with the prescriberProvide interpretation and problem solving of physician’s orders, checking for formulary status, appropriate drug selection, dose and dosage form, drug-drug interactions, drug-nutrient interactions, drug duplication, potential for adverse events, appropriate route of administration, therapeutic substitution, IV to PO conversion, or other formulary or departmental initiativesCommunicate with physician or nurse to insure prompt clarification on any part of the order in question, and recommend alternatives that are on formulary, if non-formulary medication is prescribed.Evaluate therapy for optimal patient outcomes while ensuring medication safetyCoordinate drug distribution in assigned areas.Complete or review entry of medication orders in the pharmacy computer systemPrepare/dispense medications, sterile products and compound IV admixtures and other extemporaneous orders as requested using good technique and safe practicesPerform medication storage area inspections as assignedDirect and oversee the drug distribution activities of pharmacy technicians and studentsProvide indirect supervision to technician staffParticipate in team meetings/discussions with clinical pharmacy specialists, pharmacy residents, pharmacy students and techniciansFoster an environment of teamwork with other department staff and customersServe as a preceptor for students and/or pharmacy residents on rotationProvide training and education to other staff membersProvide customer service focused team approach to internal and external customersProvide drug information and consultation to physicians, nurses, patients, and other healthcare professionals.Identify customers’ learning needsConsult with and educate customers in an effective mannerDevelop and maintain interdepartmental relationshipsAct as a primary resource for nursing staff on the proper administration of drugsProvide training and education to others as necessaryRemain current with new drug information for patient populations servedRespond to pediatric code blue emergencies as assigned to assist with drug preparation and information on emergency medications intervening as appropriate to ensure optimal patient outcomes.Participate in Medication Safety, ADE, DUE and performance improvement activitiesComplete medication safety and ADE forms when issues are identifiedComplete and follow through on Drug Usage Evaluation (DUE) forms as assignedIdentify additional opportunities to improve patient care and the performance of the pharmacy departmentMeet all role-related and age specific competencies. Comply with all initial and ongoing competencies within the entity and department. Complete required learning relevant to the role.Required:PharmD or Bachelors in PharmacyEligible for MN Pharmacy LicenseCompletion of an ASHP-accredited PGY1 residency programPreferred:Completion of a PGY2 Pediatric Pharmacy Residency or equivalent pediatric experienceBenefit OverviewAlthough this is considered a non-benefit eligible position, Fairview offers employees the option to participate in the Fairview 403(b) plan, earn Sick and Safe Time, and early access to earned wages.Compensation DisclaimerThe posted compensation range corresponds to the minimum and maximum pay rates outlined in your union agreement for the respective job classifications at the time of job posting.
