Job Description

Pharmacy Technician

Pharmacy Operations/Processing:

Workflow should be organized to complete the workload as accurately, timely, and efficiently as possible.

  1. Prescription/order processing will be performed in accordance with The Joint Commission accreditation standards, ASHP pharmacy practice standards and VHA requirements.
  2. Reviews all medication orders for appropriateness, drug selection, dosage, route of administration and the amount, prior to dispensing.
  3. Ensures all medication orders are entered into the electronic patient medical record.
  4. Provides medication counseling to patients

Quality and Patient Safety:

  1. Performs a thorough review of all prescriptions/orders processed, checked, and dispensed, uses clinical judgement when making recommendations.
  2. Processes all orders in -drug, drug-nutrient, drug-laboratory and drug-disease state interactions. Assessment for clinical relevance should be performed and dealt with appropriately.
  3. Performs continuous evaluation of prescribed medications to assure optimal drug therapy.
  4. Assesses drug safety and efficacy, including evaluation of physical symptoms.
  5. Reports adverse drug events (ADE), near misses, and medication errors in alignment with VA ADERS, JPSR, and any other pertinent reporting programs.
  6. Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and in writing.

Clinical Activities:

  1. Provides and documents continuity of care by extending refills (per policy) for patients without refills or refers patient to appropriate healthcare provider.
  2. Demonstrates a working knowledge of VHA directives, VHA national formulary, Cincinnati VA policies and protocols including activity of the Pharmacy and Therapeutics (P&T) Committee to provide appropriate and cost-effective drug therapy recommendations. Implements and monitors cost-effective drug regiments (protocols promoting positive patient outcomes).
  3. Documents patient care activities (medication reconciliation, clinical interventions, etc.) and communicates with healthcare providers consistent with Medical Center policy and procedures and Joint Commission standards.
  4. Performs medication reconciliation at all transitions of care, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications. This may include adding non-VA medications or discontinuing duplicate medications or those the patient is not taking.

Administration/Project Management:

  1. Assumes full and complete responsibilities of the pharmacy operations in the absence of a supervisory pharmacist and works independently with minimal supervisory controls.
  2. Directs and inspects the work of technicians and/or aides involved in technical functions for Pharmacy Service in the area in which assigned.
  3. Attends, participates, and contributes as an active member of various Medical Center Committees as determined by Pharmacy Service supervisory personnel.
  4. Assesses plans and evaluates the scope and quality of the pharmacy program(s) to which assigned, to ensure proper coordination between the delivery of pharmacy services and the overall delivery of healthcare to the patient.

Stock Control and Environmental Safety/Security:

  1. Manages recalls and medication shortage situations by substituting alternate dosage strengths and instructions of the same medication to equal the prescribed dose and schedule.
  2. Maintains adequate drug stock, inspects drug storage areas, supervises prepacking and the operation of automated dispensing equipment.

Educational Activities:

  1. Serves as a role model and preceptor to pharmacy students and residents. Assumes an active role in the overall pharmacy-training program consisting of students, interns, residents, and professional and non-professional staff members. Conducts on-the-job training and provides guidance through designated programs.
  2. Assumes an active role in staff development of peers and provides educational in-services to nursing, medical staff or to other ancillary services as appropriate.

Work Schedule: 3rd shift (2100-0700), rotating holidays and weekends
